Forms Assignment Due March 13, 2009
Create a web page with a form as shown in the following two graphics. The first one shows the form before any data has been entered. The second image shows the form with the drop down menu selected. The password box should accept an entry in password format. The two buttons should do what the labels state.
Use the attribute action with a value of mailto to submit the form. For the purpose of grading this assignment have the form submitted to me at sidbury@cs.uofs.edu.
