Create a survey form which collects personal and demographic information. It will also ask specific questions with possible responses with radio buttons. There should be at least three qestions. The results will be collected with a PHP script and stored in a data file whose name is form_data.dat. The form should have reset and submit buttons. The subject of the questionnaire can be any topic which interests you.
Use the attribute action with a script file in php to submit the form. For the purpose of grading this assignment, send an e-mail tosidbury@scranton.edu and attach a copy of the file form_data.dat. Make the subject of the e-mail My PHP data form.
The project must be posted on your Web site by April 25 at 11:59 p.m. Before 10:00 pm on April 29th go to the Web sites of each person in this class and fill out their form. Send the data file to me after 10:00 pm. The grade on this assignment will depend on not only your page but also on your participating on the Web sites of the other members of your class. I will attempt to fill out your form on April 26. If it is not there I will not fill it out and you will lose points. If I successfully fill out someone elses form then I expect you to do so as well.
